At Student Health Services, we offer a variety of health screenings & educational information to enhance student's learning.

Sports Sessions--Completed sports physical packets are due to the Nurse's Office by:
Fall Sports--September 8, 2023
Winter Sports--October 27, 2023
Spring Sports--March 1, 2024
***Parents--please adhere to the dates supplied as state mandates require our school physician to review and sign-off on all private examinations. Also, have your private medical home complete all parts of the physical forms. Otherwise, our school doctor will NOT sign and the forms will be returned to you. We want to help your student/athlete avoid any delays in the processing of the sports forms. Thank you in advance.
State immunization requirements for incoming 6th* graders/ 11* years old are in progress. Immunizations are not exempt during this time. Parents are encouraged to continue to have their children's regular pediatric visits and stay up to date with all immunizations per their health schedule.
Click here to obtain news and download forms for sports physical examinations. Repeater forms are also available on this site. Feel free to call the nurses' office if you have questions/ or need further information. Completed forms are considered current if physical examination is dated within 364 days to the start of the new sports session.
